1. To schedule your initial Medical Evaluation, please click the "REQUEST APPOINTMENT" button below. If you're not ready to book an appointment and have questions, don't hesitate to use the "CONTACT ME FORM" provided.
2. Before your appointment, please complete the necessary CONSENT FORMS and INTAKE FORMS. Note that the Medical Evaluation incurs a fee of $100, which is payable upfront. This Telehealth appointment is crucial for establishing an accurate diagnosis and treatment plan.
3. You will receive a message through the Client Portal outlining various options and prices for products that align with our mutually agreed-upon plan. The Prescription Order will be placed once you select an option and pay the invoice, and the prescription will be mailed directly to your home.
4. Please report your weekly weight, which should be measured prior to your weekly injection, through the Client Portal. Additionally, feel free to express any concerns through the Client Portal.
5. Every 2 to 3 weeks, we will discuss your progress and collaboratively determine the next best steps. We will place the appropriate prescription order once the invoice is paid.
